It is fluent, confident, and wrong

Ungrounded generation over internal data produces plausible unsourced answers. The correction is retrieval with citations and an acceptable refusal, not a longer instruction.

It crosses a boundary nobody noticed

One shared index means anyone who can ask can reach anything indexed. Access control belongs at retrieval, inheriting rules that already exist.

It worked on ten documents

A curated pilot corpus behaves nothing like the real estate: duplicates, scans, superseded versions, contradictions. That gap is closed with chunking, hybrid search and reranking, not a better model.

Nobody can tell whether it degraded

Without evaluation every change is a guess and every regression is discovered by whoever it damages first.

Retrieval enforces the access rules the host system already holds. An agent cannot surface a document the person asking was never entitled to open.

Open-weight models on infrastructure you control, where data residency or provider exposure is the binding constraint rather than a preference.

A test set with expected behavior, run on every change. Without it, nobody can distinguish an improvement from a regression that has not surfaced yet.
